You will complete a few questionnaires about lifestyle, health, diet, and environment that will take about 40-60 minutes. Some of these questionnaires will be completed using a touch screen. Others will be done on paper. If you need assistance with the questionnaires, a staff member will be on hand to help.
You can reduce the length of your Assessment Centre visit by completing some of the questionnaires at home before you arrive and bringing these with you to your appointment. We can include the questionnaires as part of the invitation package we mail you, or you can click on the link below to download them.
